FMCSA Insurance Filing Help
File Certificate
1.) Identify the carrier. If you don't know either the US DOT number or the docket number, open the Carrier Search page to identify them and then return to the File Certificate - Carrier Identification page.
2.) Identify the filer (if there is more than one option) and the type of certificate for the filing, and then click CONTINUE FILING.
The types of certificates are: BIPD-91X Primary, BIPD-91X Excess, BIPD-91X Full 1,
BIPD-91X Full 2, BIPD-91, Bond-82, Bond-83, Bond-84, Trust-85, Cargo-34.
3.) Enter the effective date and the policy number. If the certificate type is 91X Primary, enter the maximum coverage too.
If the certificate type is 91X Excess, enter both the maximum and the underlying coverage.
Click SUBMIT to continue. If the required fields are blank or invalid the system will display the entry-screen again so you can correct the data.
4.) Check the data you entered. The system issues a warning if the effective date is more than 30 days in the past or more than 90 days in the future.
Since the common coverage amounts are 750, 1000, 1500, and 5000, a warning will appear if the maximum coverage is not one of these amounts.
Click BACK to re-enter incorrect data or click SUBMIT to add the record to the batch.
5.) The confirmation page shows which records are being submitted to the system. You can also file additional certificates, cancel certificates, see the entries for the current batch, or process the batch from this page.
Note: Filings will not be posted to the database until the
batch is processed.
