FMCSA Insurance Filing (Cancellation) Help
Cancel Certificate
1.) Identify the carrier. If you don't know either the US DOT number or the docket number, open the Carrier Search page to find them, and then return to the Carrier Identification - Cancel Certificatepage.
2.) Identify the certificate that is to be cancelled (from the list of certificates). Click SELECT on that line. This list has all of the certificates for the carrier for any filer associated with
the login ID.
If a certificate has already been marked as cancelled it will not be displayed.
3.) Enter the date when the cancellation will take effect. The date must be more than 30 days in the future. The effective date is 30 days after the record is processed, rather than the cancellation date. If a certificate is cancelled but the batch it is in has not been processed, the effective date will be 30 days from the processed date.
Click SUBMIT to add the cancellation to the batch as valid. If the date is invalid or if it is less than 30 days in the future the system will display the entry-screen again.
4.) The confirmation page displays the data that are being submitted to the system. You may also file or cancel certificates, review entries for the current batch, or process the batch from this page.
