Dismissals
Click DOCUMENT for the dismissal letter sent to carriers that did not provide required insurance and process agents by the 20th day after publication of their authority in the FMCSA Register.

Dismissals (OP-1[MX] / OP-2)
Click DOCUMENT to display the dismissal letter sent to Mexican carriers that did not comply with regulations. Their authority will be dismissed 60 days after the published dismissal date unless they achieve compliance before then.

MX Notice of Proposed Suspension

MX Order to Show Cause
Click DOCUMENT for the letter notifying Mexican carriers that violated safety regulations while suspended, and before revocation took effect, of their imminent revocation.

MX Revocation Letter
Click DOCUMENT to display the letter notifying Mexican carriers that received unsatisfactory ratings on their safety monitoring and have not performed the required corrective action that their authority will be revoked within a specified time period.

MX SA No-Contact Letter
Click DOCUMENT to display the letter notifying Mexican carriers that they could not be reached to schedule a safety audit and that their provisional operating authority will be suspended in 30 days if they do not contact the FMCSA.

MX Suspension Letter
Click DOCUMENT to display the letter notifying Mexican carriers that failed their safety audit that their authority has been suspended.

MX Unable To Contact (3 attempts) Letter
Click DOCUMENT to display the letter notifying Mexican carriers that could not be reached to schedule a safety audit that their provisional operating authority will be suspended in 15 days and that they will be placed out of service if they do not contact the FMCSA.

Name Changes
Click DOCUMENT to see the letter sent to carriers that changed their names. These carriers must submit new insurance and pay a new application fee.

Temporary Emergency Authority Letters
Click DOCUMENT to see the temporary emergency authority that is granted to INTRASTATE carriers that wish to have a temporary INTERSTATE authority in the event of a national disaster. This authority is valid for 120 days from the service date (which is the same as the decision date).

Transfer Certificates, Permits, Licenses
Click DOCUMENT for the authority that allows carriers that have transferred their operation or have merged to continue providing transportation services.

Transfer Dismissals
Click DOCUMENT to display the letter dismissing the authority of carriers that did not submit new insurance and BOC-3 forms after transferring all or part of their operation. Carriers must comply within 30 days to stop the dismissal.

Transfer Grant Letters
Click DOCUMENT for the new CPL letter sent to carriers that have transferred all or part of their authority, and have complied with insurance and BOC-3 requirements.

Ad hoc Reports
Authorized users may run ad hoc reports.
You must confirm the type of report that you choose. To proceed, click OK. On the second confirmation page, enter the release date for the document or list and then click SUBMIT to run it.

The reports which users may define are:

Application Proof List
The application proof list identifies applicants and their filing dates and company representatives.

Authority Anomalies
Enter selection criteria to find carriers with generic authority(common, contract, and broker) that requires(ed) the generic type to be made specific(Motor Property Common, etc.).

Authority Application Fee Refund
View a list of carriers due refunds based on the SAFETEA-LU Requirements for common and contract authority.

Enterprise Carrier Statistics
View a list of carriers based in the United States that are owned by Mexican citizens.

MX OP-1 Application Logged Status
View a report of Mexican long-haul (OP-1[MX]) applications by status type within a specified period.

MX OP Application Activity Statistics
View the number of Mexican applications by status type within a specified period of time.

Non-Fitness / Fitness List
View all applications from carriers that have been protested as unfit to provide service.

Register
View upcoming FMCSA Registers.

Registration Credit Card Transactions
Pick Registration Credit Card Transactions from the drop-down list and then click GO to enter the purchaser ID on the Registration Credit Card Transaction Selection Criteria page and display the credit card payment data for that carrier.

Rejected Applications List
View which applications have been rejected because the carrier filed an incomplete application or if the information was entered incorrectly.

Transfer Application Current Status Statistics
View the total number of transfer applications by their status.

Transfer Application Statistics by Date
View the total number of transfer applications by status type within a specific period.

User Case List
View applications by user.

Schedule a Pin Letter

Schedule a mailing of a carrier's existing L&I Docket Pin Number or request a new one to be mailed to the carrier.


View Pin Letters generated in a time range, both Docket and DOT Number Pin Letters. Click Pin Letter File Name to read the pdf of letters for a given date.


FMCSA Register - Delete Documents

Supervisors may delete documents from the FMCSA Register by clicking GO next to DELETE DOCUMENT AND REGISTER ENTRY on the Insurance Document page. On the selection criteria page, specify the date, type of document and docket number for the documents you want to remove and then press DELETE.
