Motor Carrier Details page
US DOT Number
The number assigned to a census record in the Motor Carrier Management Information
System (MCMIS).
Each motor carrier should have only one active census number.
The census number has no internally coded structure. Numbers are issued sequentially as carriers are added to the system.
Note: If the carrier has been placed out of service the letters 'OOS' appear beneath the US DOT number. When you display the motor carrier details (either in HTML or PDF formats), the following sentence appears above the Doing-Business-As Name field: 'This carrier is out of service.'
Docket Number
The docket number is assigned to motor carriers engaging in interstate or foreign operations. It consists of the prefix and the numbers that are issued sequentially as carriers are registered in the system.
The docket number is found in the Licensing and Insurance (L&I) database of the Federal Motor Carrier Safety Administration (FMCSA).
Note: Prior to January 1, 1996, docket numbers were assigned by the Interstate Commerce by the Commission (ICC).
Most numbers are prefixed by "MC", which stands for 'motor carrier.'
Other prefixes are "MX" for a motor carrier that is owned or controlled by a citizen of Mexico or a person domiciled in Mexico, and "FF", for a freight forwarder.
Legal Name
The legal name of the motor carrier.
Business Address
The address where the motor carrier's principal place of business is physically located.
Business Telephone and Fax
The motor carrier's telephone and fax numbers at the principal place of business. For most carriers, only the telephone number has been provided.
Mail Address
An alternative address for motor carriers whose mailing address is different from their principal place of business.
Mail Telephone and Fax
The telephone and fax number for motor carriers whose mailing address is different from their principal place of business.
Undeliverable Mail
A Yes/No field that states whether mail sent to the carrier's address was returned.
Mexican-Owned
A Yes/No field that denotes whether a carrier is domiciled in Mexico or is owned or controlled by a Mexican citizen.
OP-2 Zone
The areas of operation for OP-2 carriers. They include:

Application Type
The type of application that was submitted after NAFTA was passed. This is either OP-2 or OP-1[MX].
NOTE: A carrier may have only one type of authority. Carriers that want to engage in long-haul transportation as well as business in the border commercial zones must apply for the OP-1[MX].
Authorities
An 'Active' status for the Common, Contract, or Broker, fields means the carrier, freight forwarder, and/or broker is currently registered and has complied with applicable insurance regulations.
An 'Inactive' status means that the operating authority for the carrier, freight forwarder, and/or broker has been revoked and for-hire interstate operations are illegal.
A 'None' status means the carrier, freight forwarder, and/or broker has not registered for that type of authority.
A 'Yes' in the Application Pending and Revocation Pending fields indicates a pending action that will affect the current authority (or authorities).
The Property, Passenger, Household Goods, Private, and Enterprise fields indicate the type of cargo or persons that the carrier is authorized to transport.
NOTE:A private carrier may not be an enterprise carrier or a passenger carrier as well. Otherwise the classifications are not mutually exclusive.

Insurance Requirements
'BIPD' refers to bodily injury and property damage liability insurance.
'Cargo' refers to motor carrier cargo liability insurance.
'Bond' refers to surety bonds or trust funds that must be maintained for the carrier by authorized brokers and freight forwarders.
If bodily injury and property damage liability insurance is required, the dollar amount of insurance that is required, and the dollar amount that is on file, are displayed.
For cargo and bond, 'Yes' in the Required field indicates that that type of coverage is required.
A 'Yes' status means that the carrier, freight forwarder, or property broker is in compliance.
A 'No' status means that the carrier, freight forwarder, or property broker either does not have an active authority, or is not in compliance and may be subject to revocation proceedings. Operating while not in compliance with federal regulations is unlawful.
If the answer in the BOC-3 field is 'Yes', it is followed by the name of the blanket company in the Blanket Company field, if that name is in the database.
Active/Pending Insurance
This displays the details of all active or pending insurance for the carrier that is on file with the FMCSA: the form the insurance was filed on, the type of insurance, the insurance carrier, the policy/surety number, the date when the insurance was entered in the L&I system, the coverage of the policy, the effective date of the policy and, in the case of pending cancellations, the cancellation date.
For cargo and surety bonds, if a carrier is in compliance, the amount of coverage will always be shown as the required federal minimum ($5,000 per vehicle, $10,000 per occurrence for cargo insurance, $75,000 for bond/trust fund insurance for brokers and freight forwarders). The carrier may actually have higher levels of coverage.
Rejected Insurance
This displays the rejected insurance filed with the FMCSA during the prior 12 months: the form the insurance was filed on, the type of insurance, the insurance carrier, the policy/surety number, the coverage of the policy, the receipt date of the filing and the reason(s) the filing was rejected.
For cargo and surety bonds, if a carrier is in compliance, the amount of coverage will always be shown as the required federal minimum ($5,000 per vehicle, $10,000 per occurrence for cargo insurance, $75,000 for bond/trust fund insurance for brokers and freight forwarders). The carrier may actually have higher levels of coverage.
Insurance History
This displays the details of all historical insurance for the carrier that was on file with the FMCSA since 1995: the form the insurance was filed on, the type of insurance, the insurance carrier, the policy/surety number, the date when the insurance was entered in the L&I system, the coverage of the policy, the effective date of the policy and, in the case of pending cancellations, the cancellation date.

Authority History
This displays all of the activity that has occurred for a carrier's authority: the authority type, the date and action taken of the original activity and the disposition date and action, if applicable.
In the case of older carriers, not all of the history has been added to the database.
Pending Application
This displays any pending applications that a carrier has filed, but that have not yet been granted, denied, or dismissed.
The following details are shown: the authority type, the date filed, the status of the application, and the status of compliance with insurance and BOC-3 requirements.
'OK' in the Insurance or BOC-3 fields indicates compliance and 'Not OK' indicates that the carrier is not yet in compliance.
Revocation
This displays all revocation activity for the carrier: the type of authority, the service dates, and the reason for the revocation.
NOTICE
The Federal Motor Carrier Safety Administration (FMCSA) is notifying the public that the time-frame that is required in order to revoke a carrier's operating authority for failing to have sufficient levels of insurance on file is being reduced.
The new 33-day process will allow a carrier to only hold operating authority without insurance reflected on the Licensing & Insurance database for up to three (3) days.
Decisions on revocations will be tied to our enforcement program, which will focus on the operations of uninsured carriers. This improvement will further ensure that the public is adequately protected in case of a motor carrier crash.
