When you opened the Form MCS-150 page on the FMCSA web site you were asked to enter a credit card number for your company (this may be a corporate business card, or an owner's personal card, for example).
The credit card and personal information you enter on this page act as your signature and certification that you are familiar with the applicable Federal Motor Carrier Safety Regulations and/or the Federal Hazardous Materials Regulations.
This procedure is only for identification purposes. There is no fee to apply for a U.S. DOT number, and no charge will be made to the credit card you use when you file a Form MCS-150.
Note: All yellow fields with an asterisk (*) next to their label-name are mandatory fields.
Applying for your U.S. DOT Number:
Step 1: Enter or select data in all the fields on the Add MCS-150 page and click CONTINUE at the bottom of the page.
Note: Click CLEAR at the bottom of the page to erase the data you have entered on this page and start over.
Step 2: Enter or select data in all the fields on the Continue MCS-150 Add page and then click ADD at the bottom of the page to apply for your U.S. DOT number.
